Peanut Pal’s annual membership runs from October 1, 2012 through September 30, 2013
Annual Membership Dues are due on or before October 1, 2012
Annual dues are $20.00 for Primary Members for the period October 1, 2012 through September 30, 2013 . Associate memberships are available for other individuals of the Primary Member’s family who wish to join PEANUT PALS but not receive additional newsletters or other mailings. Annual dues for Associate Members are: $10.00 per person 16 or older and $3.00 for those under the age of 16.

  Please complete the form below – Dues may be renewed for ONE YEAR ONLY – all Associate Members must renew for the same amount of time as the Primary Member.

2. Fill in the appropriate information on the Membership Application.

3. PRINT 2 COPIES OF COMPLETED MEMBERSHIP APPLICATION:
   One copy to be mailed with your payment AND one to be retained
   for your records.

4. Mail completed Membership Application and Payment to address
   shown on application.

YOU MUST DOWNLOAD AND SAVE THIS MEMBERSHIP APPLICATION ON YOUR COMPUTER !

1. Click on Membership Application at right to Open, SAVE A COPY
    OF THE MEMBERSHIP APPLICATION TO YOUR COMPUTER.

2. Open the Membership Application you saved on YOUR COMPUTER.
   Fill in the appropriate information on the Membership Application.

3. E-mail Membership Application to address shown on application.
